Copay Plan is part of a Preferred Provider Organization (PPO) network and offers easy access to
everyday care with low office visit copays and convenience care copays when using an in-network

Doctor on Demand (DOD) is available to all employees enrolled in the CSM medical plan. DOD is the fastest, easiest way to see a doctor anywhere or anytime on your computer, tablet or smart phone – from the comfort of your own home 24 hours a day, seven days a week. You can download the app by searching “Doctor on Demand” in the Apple App Store or Google Play or visit doctorondemand.com to get started.
How much does it cost? DOD is FREE for those enrolled in the CSM medical plan.
